Community Care Network (CCN) – Supporting Veterans’ Health Needs
The Community Care Network (CCN) was established to ensure Veterans receive timely, high-quality health care. BrightStar Care, a VA-credentialed provider, works closely with eligible Veterans and their primary care providers to deliver home care services.
Each VA office has its own eligibility guidelines, but typically, to qualify for Homemaker and Home Health Aide Care (H/HHA), a Veteran must require assistance with at least three Activities of Daily Living (ADLs).

- HHAs (also called caregivers) are not nurses but provide essential support.
- They work under the supervision of a Registered Nurse (RN) who regularly assesses the Veteran’s care needs.

VA Aid and Attendance & Housebound Benefits
While Community Care Network (CCN) benefits are paid directly to providers, the VA Aid and Attendance and Housebound Allowance programs provide monthly financial assistance to eligible Veterans and survivors who receive a VA pension.
Who Qualifies for VA Aid and Attendance?
If you already receive a VA pension, you may be eligible if you meet at least one of the following criteria:
✔️ You need help with daily activities (bathing, dressing, feeding, etc.).
✔️ You must stay in bed or spend most of the day in bed due to illness.
✔️ You are a nursing home patient due to disability-related mental or physical limitations.
✔️ You have severely limited eyesight (5/200 vision or less, or a contracted visual field of 5 degrees or less).
Who Qualifies for Housebound Benefits?
You may be eligible if you receive a VA pension and spend most of your time at home due to a permanent disability.
Some Veterans may qualify for both benefits, so it’s essential to check your eligibility.
VA Health Coverage for Skilled Services
While CCN and VA Aid and Attendance benefits help cover Activities of Daily Living (ADLs) for eligible Veterans and survivors, VA Health Care may provide coverage for skilled home care services not covered by Medicare.
Skilled Services for Veterans
Nurses can offer a variety of skilled services at home to meet the medical needs of Veterans, including:

✔️ Ostomy and catheter care
✔️ Infusions and blood draws
✔️ Other approved treatments
Therapies at Home
Veterans with VA Health may also be eligible to receive Physical Therapy (PT), Occupational Therapy (OT), and Speech Therapy (SLP) at home. These therapies aim to help individuals:

✔️ Perform daily activities after an illness, injury, or surgery
Speech and language pathologists can assist patients recovering from conditions like stroke, focusing on rehabilitation for issues such as:
✔️ Swallowing
✔️ Speaking

Where Do I Start?
The VA provides valuable resources to help you navigate the eligibility determination process and application procedures. While we’ve included direct links and initial information to help you get started, we’re happy to guide you step by step throughout the process.
There are numerous details and nuances that can impact the timeline for receiving services. Our experience assisting other Veterans makes us a great resource to ensure you avoid delays and get the services you need without hassle.
To begin, complete the VA Form 21-2680 (Examination for Housebound Status or Permanent Need for Regular Aid and Attendance) and mail it to the VA Pension Management Center (PMC) for your state. You can also have your doctor fill out the medical section of this form. If needed, you can submit additional evidence along with your form.
If you’re currently residing in a nursing home, you’ll also need to fill out the Request for Nursing Home Information in Connection with Claim for Aid and Attendance (VA Form 21-0779).
For those who prefer in-person applications, you can take the required documents to your local Veterans Affairs regional office, which can be located here.
Once your application is complete, contact your nearest VA facility to arrange an assessment for requesting Home Health Aide (HHA) or Respite Care services. When making this request, you can specify that you want LNT Health as your service provider, as we are credentialed to provide care for Veterans.
After your assessment, a VA case manager will inform you of the number of care hours you’re eligible for and which provider has been authorized to deliver the care. You can always request LNT Health as your preferred provider at any time. If necessary, the case manager will coordinate with our home care agency to fulfill your needs.
